Most interesting is a user story[1] shared giving an idea of how often this happens: 3 times over 8 months, with ~5 calls a week. We could round it at around 1% error rate. It's both relatively low (as a chance it happens to you), and extremely high (I can't imagine how many people might have the issue and not be aware of it!).

I'm sure there are other conditions that aren't identified here, but this definitely warrants more regulatory scrutiny.
